Each AFR Forms zip file contains forms and instructions for completing the school district AFR, including the School-Level Reporting form and Food service AFR; Classroom Site Fund (CSF) Narrative; Work Sheet for Determining the Appearance of Supplanting with CTED Monies; and ADE’s Results-Based Funding Expenditure Report. The Desegregation AFR Form zip file contains the form and instructions for districts that budget for desegregation. A.R.S. §15-904(F) requires all districts, including those that operate only 1 school, to complete a school-level report.
Select the zip file that corresponds to the number of schools in your district and extract the files to C:\SDFORMS\. In addition to the extracted files, the C:\SDFORMS\ folder should also include the most recently revised versions of the district's FY 2021 budget and FY 2020 AFR files. Career and Technical Education Districts with only 1 centralized campus must still use the form for 2-25 schools.
