FY 2025 School District Annual Financial Report Package

The FY 2025 AFR zip file contains forms and instructions for completing the school district AFR; 2 school-level AFR forms, based on district size; Classroom Site Fund (CSF) Narrative; and Desegregation AFR for districts that budget for desegregation monies.

The FY 2025 AFR packet also includes the Work Sheet for Determining the Appearance of Supplanting with CTED Monies. Member districts are required to submit the completed form to the CTED’s governing board and the Arizona Department of Education.

Select the zip file and extract the files to C:\SDFORMS\. Choose the appropriate school-level AFR file based on the number of schools in your district. In addition to the extracted files, the C:\SDFORMS\ folder should also include the most recently revised versions of the district's FY 2025 budget, FY 2024 AFR, and FY 2024 Food Service AFR files.
